This completed application should be returned to the school along with the following items:

• $50 examination/administration fee (non-refundable)

• photocopy of applicant’s Certificate of Eligibility for Education in English

• photocopy of applicant’s birth certificate showing both parents’ names

• photocopy of applicant’s final report card of previous school year

• small photo of applicant
